The Wonderful Use of Zhengan Xifeng Decoction (Part 2)
Coronary Heart Disease, Angina Pectoris
Coronary heart disease, also known as ischemic heart disease, is a coronary artery atherosclerosis heart disease; a general term for functional changes in the coronary arteries. Angina pectoris is a clinical syndrome caused by acute and temporary ischemia and hypoxia due to inadequate coronary artery blood supply. Its characteristics include paroxysmal squeezing chest pain, mainly in the posterior sternum, radiating to the precordial region and left upper limb. It often occurs during labor or emotional excitement, lasts for several minutes, and disappears after rest or the use of nitrates. The pathogenesis is related to myocardial ischemia and hypoxia. Diagnosis is mainly based on clinical manifestations and laboratory examinations, such as ST segment depression and T wave inversion on electrocardiogram during attacks.
Coronary heart disease and angina pectoris belong to the category of "chest obstruction and heart pain" in traditional Chinese medicine, with periods of attack and remission. The disease is often a mixture of deficiency and excess, with excess predominating during the attack period and deficiency predominating during the remission period. The treatment should aim to tonify the middle and promote circulation, promote circulation while tonifying the middle, and tonify and promote circulation simultaneously. Zhengan Xifeng Decoction is suitable for the remission period.
Modification of Zhengan Xifeng Decoction: 12g of white peony, 12g of scrophularia, 30g of cinnabar (pre-decocted), 18g of achyranthes root, 6g of gentian grass, 30g of musk mallow, 15g of mulberry mistletoe, 30g of hook vine (added later), 9g of sichuan chinaberry, 3g of antelope horn (powdered and taken with water), 5 scorpions, 6g of licorice root. Decoct in water, divide into 2 servings, warm to take twice a day, 1 dose per day.
For those with heart irritability and red tip of the tongue and little saliva, add schisandra chinensis and sour jujube kernel to nourish the heart and calm the mind; for those with fatigue, weakness, and poor appetite, use pinellia ternata to invigorate the spleen and harmonize the stomach, and use polygala and polygala tenuifolia to tonify the heart and calm the mind.

Prescription: 20g Codonopsis pilosula, 15g scrophularia, 15g ophiopogon japonicus, 15g white peony, 18g dragon bone, 12g melon seed peel, 20g cinnabar (pre-decocted), 30g oyster shell, 30g turtle shell (pre-decocted), 30g kudzu root. 1 dose per day, decoct in water and take in 3 divided doses, with symptomatic treatment for 1 month. The symptoms basically disappeared. It is recommended to continue with tonifying the heart and invigorating qi and calming the mind to consolidate the therapeutic effect.
(2) According to "Chinese Medicine Information", 1997, (5):26, Zou Yaoguang et al. reported: Zhengan Xifeng Decoction has satisfactory efficacy in the treatment of chest obstruction and heart pain in the elderly. Prescription: 15g white peony, 15g ophiopogon japonicus, 15g scrophularia, 10g Codonopsis pilosula, 30g melon seed peel, 15g dragon bone (pre-decocted), 15g cinnabar (pre-decocted), 15g oyster shell (pre-decocted), 30g turtle shell (pre-decocted), 1 dose per day, taken twice daily. It is recommended to consolidate the therapeutic effect with methods to tonify the heart and calm the mind.
